Brain aneurysm: Clipping or coiling? One of the most common questions I hear is whether a brain aneurysm should be treated by surgical clipping or endovascular coiling. The answer is that there is no single treatment that is best for every patient. The choice depends on factors such as whether the aneurysm has ruptured, its size, shape and location, the patient's age, medical condition, and long-term treatment goals. Current international guidelines generally favor endovascular coiling for many aneurysms when it is technically feasible. An important exception is stent-assisted coiling, where the need for prolonged dual antiplatelet therapy may make it less suitable in certain situations, particularly in the setting of a ruptured aneurysm. Many patients are concerned about cost. While the procedure cost of coiling may be slightly higher, the shorter hospital stay, faster recovery, and earlier return to daily activities often make the overall cost difference between clipping and coiling much smaller than expected. As a neurosurgeon trained in both microsurgical clipping and endovascular aneurysm treatment, I individualize every treatment decision based on the patient's aneurysm characteristics, overall clinical condition, and current evidence.
