What Happens in Chronic Venous Insufficiency (CVI)? Chronic Venous Insufficiency occurs when the veins in the legs fail to efficiently return blood back to the heart. Normally, one-way valves in the veins prevent backward flow. When these valves become weak or damaged, blood starts pooling in the legs — a condition known as venous reflux. Over time, this increased pressure in the veins (venous hypertension) leads to visible and symptomatic changes. Patients may experience leg heaviness, aching pain, swelling (especially around the ankles), and fatigue, which typically worsen after prolonged standing. As the condition progresses, skin changes such as darkening (hyperpigmentation), itching, and thickening may develop. In advanced stages, non-healing ulcers can occur, particularly near the ankle region. Early diagnosis and treatment can prevent complications and significantly improve quality of life.
